Limit your contact information by removing your middle name, home address, phone number, and any email address that can be easily associated with an online profile.
Instead, include an unlisted cell phone number and a generic email address.
Remove any dates or specific license/certificate numbers from your education section.
Remove your employer's name and replace it with a generic but accurate description. For example, Grand Rapids Hospital could become simply Major Hospital.
You might also consider changing unique job titles if they are unusual and could be easily used to identify you.
